THE FIRST VISIT
Is this your child's FIRST visit to see us? Below you'll find the "New Patient Package" if you'd like to print it out and have it completed. This will speed up your visit and get your child seen quicker. NOTE: be sure to print out ALL pages in this excel spreadsheet.
What can you expect when you get here? You'll come in, sign-in and be sent back to an exam room. The nurse will come in and ask many questions, get vital signs, and prepare the chart for the doctor. When he has reviewed your child's chart, the doctor will come in and verify the history that the nurse recorded, examine your child and tell you the medical plan. He will then go back to his office while you wait where he will finalize the information, calculate medication doses (if needed) and return to release you. That's it.
